Step One - Select the Services you Desire

  • Select a pre-packaged program from a customized menu.
  • Add a-la-carte services from a customized list.
  • Order multiple requests with a touch of a button.


Step Two - Complete Enrollment Form

  • Provide name and contact of the donor or subject.
  • Add additional contact information for ease of communication.
  • Track the reason for your request; i.e.  pre-employment, periodic, post accident.
  • Add a reference code to track billing or location code.

Step Three - Clinic Selection

  • Select your clinic location based on zipcode and desired location radius.
  • Clinic availability information includes: days and hours of operation.
  • Easy to navigate mapping feature to help guide you along with downloadable maps.
  • Enhanced contact information displays clinic phone, fax, email and web information, if available.
  • Specific site information and additional comments displayed, if available.

 

 Step Four - Patient Service Center (PSC) Selection

  • Select your PSC location based on zipcode and desired location radius.
  • PSC availability information includes: days and hours of operation.
  • Easy to navigate mapping feature to help guide you along with downloadable maps.
  • Enhanced contact information displays clinic phone, fax, email and web information, if available.
  • Specific site information and additional comments displayed, if available.

Step Five
- Selection Preview and Request Generation

  • Preview screen displays selected criteria.
  • Ability to modify, edit or cancel request before official automated request is generated.
  • Ability to generate an authorization form for subject's signature.
  • Store, download or forward electronic generated requests as necessary.
  • Create and track log files of current and archived requests.
  • Medical Review Officer (MRO) information displayed.
  • Includes specific instructions on sending results.
