Step One - Select the Services you Desire
- Select a pre-packaged program from a customized menu.
- Add a-la-carte services from a customized list.
- Order multiple requests with a touch of a button.
Step Two - Complete Enrollment Form
- Provide name and contact of the donor or subject.
- Add additional contact information for ease of communication.
- Track the reason for your request; i.e. pre-employment, periodic, post accident.
- Add a reference code to track billing or location code.
Step Three - Clinic Selection
- Select your clinic location based on zipcode and desired location radius.
- Clinic availability information includes: days and hours of operation.
- Easy to navigate mapping feature to help guide you along with downloadable maps.
- Enhanced contact information displays clinic phone, fax, email and web information, if available.
- Specific site information and additional comments displayed, if available.
Step Four - Patient Service Center (PSC) Selection
Step Five - Selection Preview and Request Generation
- Preview screen displays selected criteria.
- Ability to modify, edit or cancel request before official automated request is generated.
- Ability to generate an authorization form for subject's signature.
- Store, download or forward electronic generated requests as necessary.
- Create and track log files of current and archived requests.
- Medical Review Officer (MRO) information displayed.
- Includes specific instructions on sending results.
