If entering request by remote or the web
Please be sure to fax over the Authorization for Release of Information to 410-356-8604 as many employers require a signature before releasing any employment information.

If faxing request
Please be sure to include the application and/or resume and ensure that applicant has signed the release.

May we Contact?
Please ensure that client and/or the applicant have clearly indicated whether or not the current employer may be contacted, especially if only a resume is being sent over.

Telephone Numbers
Please provide phone number of employment reference, or if unavailable, at least provide the city and state of employment.

Company's Full Name
Provide full name of company, not just acronym. Example: Employment Background Investigations instead of EBI.

Dates
Provide dates of employment.

Supervisor's Name
If needing extended reference check, provide supervisors name and number, if available.

Other Names Used
Provide any other names that applicant may have used, such as maiden names.

Tracer and Credit Reports

Required Information
Please ensure that the social security number and date of birth is complete and legible.

Education Verification

If entering request by remote or via the web

Full Campus Name
Provide full campus name and location including city and state. Many colleges have numerous campuses, so this will help narrow down the search.

Date of Attendance
Provide dates of attendance or at least date of graduation.

Degreed?
Please indicate whether or not the applicant has indicated receiving a degree and the type attempted or obtained.

Criminal Record Searches

Date of Birth
Provide full date of birth. Most court records in the United States are indexed by name and date of birth and do NOT list social security numbers.

Legible
Ensure that applicant has written the name legibly and you have keyed the name properly.

Motor Vehicle Requests

Proper State
Please be sure that applicant provided the State where drivers license is current.

Proper Format
Double check that applicant provided correct format of their driver's license number.
